For Better Heart Care, Get a Pharmacist on Your Team
Canadian study highlights a potential opportunity to improve public health

Thursday, June 16, 2016

WEDNESDAY, June 15, 2016 (HealthDay News) -- People with poorly controlled risk factors for heart disease could cut their chances of future trouble by having pharmacists help manage their care, new Canadian research suggests.
For the study, trained community pharmacists recruited people at high risk for heart attack and stroke. Half of the study participants received "medication therapy management" in tandem with a pharmacist and half received "usual" care.
After three months, people who received intensive services to help them meet treatment targets had a 21 percent lower risk of future heart events when compared with those who received usual care, the study found.
People receiving pharmacists care lowered their estimated future risk of heart disease by more than 5 percent from the beginning of the study to its conclusion three months later. There was little change in risk for those receiving usual care.
"Since the risk for [heart] events is so high, any reduction is a good one," said study author Ross Tsuyuki, a professor of medicine at the University of Alberta. He added that "it is likely that we have underestimated the real reduction in risk," since many risk factors for heart disease change slowly.
The study focused on reaching out to people with multiple risk factors, consulting with them, prescribing needed medicines and adjusting medication dosages.
Tsuyuki and his team believe the study may be the first large randomized trial of its kind.
Since cardiovascular disease is the leading cause of death in the United States and worldwide, such an intervention could have major public health benefits, the study authors concluded.
The report is published in the June 21 issue of the Journal of the American College of Cardiology.
Dr. Larry Weinrauch, a cardiologist and assistant professor at Harvard Medical School in Boston, said the study highlights problems with health care delivery, particularly for high-risk patients taking multiple medications.
"In the U.S., I think we ought to be looking at systems for picking up those patients [and delivering] better care to them," he said.
Whether pharmacists or other health practitioners provide that care is less important, said Weinrauch, who wrote an editorial accompanying the study.
Tsuyuki said that while it will take "political will" to make the necessary changes, the United States could benefit from replicating this model of care.
"You could have about 400,000 pharmacists available to help with your number one killer," he said.
Heart disease costs the United States an estimated $444 billion annually in health-care costs and lost productivity, the study authors noted.
Some U.S. pharmacists already manage people with chronic conditions and complex medication regimens. But its not routine because reimbursement opportunities are limited. Pharmacists, unlike doctors, are not considered "providers" of care under the federal Medicare statute and cannot bill the health program directly, the researchers said.
By contrast, pharmacists in Alberta, Canada, get reimbursed by the provincial health plan $100 to $120 a year for a comprehensive medication review and $20 to $25 for follow-up visits, Tsuyuki said.
For the study, pharmacists at 56 pharmacy-based clinics recruited 723 patients, who were randomly assigned to the intervention or usual care.
Some large and clinically significant improvements in individual risk factors emerged in the pharmacist-care group compared with the usual care control group, Tsuyuki noted.
People with high blood pressure had a 9.4 mm Hg reduction in systolic blood pressure -- the top number in a blood pressure reading. An optimal systolic reading is 120 mm Hg or less. But, only 28 percent of the control group reached the recommended target, versus 51 percent of the pharmacist-care group, the study found.
Patients with diabetes in the pharmacist-care group dropped their hemoglobin A1c -- an indicator of blood sugar levels -- from 8.6 percent at the start of the study to 7.6 percent three months later. The goal is a reading of 7 percent or less. But only 25 percent of the control group achieved it, compared with 42 percent of patients receiving pharmacist care, the investigators found.
In addition, significantly more patients receiving pharmacist care reached the target for LDL, or "bad" cholesterol, than those in the control group (56 percent versus 46 percent).
The pharmacist-care group also had a 20 percent greater reduction in smoking compared to the control group, according to the report.
One limitation of the study was its short duration; pharmacists expressed concern over continuing to provide usual care to high-risk patients, the researchers said. After three months, control group members were invited to participate in the pharmacist-care program.

How do Dogs Get Worms
How do Dogs Get Worms
There is no doubt about the fact that dogs are among the most popular pet animals. While, adopting a pet may sound simple for you, the efforts behind taking care of the animal may not be that easy. Apart from providing the right diet, shelter and other things, you must also take care of its health problems. As far as canines are concerned, internal parasites or dog worms are found to be a common health problem. So, it is not really a bad idea for the dog owners (including the prospective ones) to gain some knowledge about this canine health problem.
Types of Dog Worms
Internal parasites are those organisms that live inside the body of the host, from whom they derive their food. In case of dogs, there are many different types of internal parasites that are called dog worms. The most common among them are roundworms, hookworms, whipworms, tapeworms and heartworms. While heartworms are found to occupy the heart and the blood vessels, the other four infest the intestines. While these parasites can be found in dogs of all ages, roundworms and hookworms are prominently seen in puppies, whereas, the other three (heartworms, tapeworms and whipworms) are mostly seen in older canines.
Some of the dog worms, like, tapeworms are visible, some others are microscopic in nature. In small numbers, these parasites may not cause any dog worm symptoms, but, large infestation may prove detrimental for the health of the animal. Another problem that may be posed by dog worms is the risk of human infection. Roundworms, hookworms and tapeworms in dogs may get transmitted to people. So, preventing and treating dog worms is of utmost importance.
How do Dogs Get Worms
There are many different ways through which a dog can get infested with worms. Such dog worm transmission may take place right before the birth of the animal or after that. The following are some of the common ways of dog worm transmission.
Roundworms: One of the commonly found dog worms, roundworms look like cooked spaghetti. There are many different types of roundworms and Toxocara canis is mostly found in dogs. These worms are usually found in the intestine of the dog, but, they may travel through the blood to the liver, lungs and the brain as well. Newborn puppies are found to be infested with these parasites, during the fetal stage. In other words, an infected mother can transmit these parasites to the fetus in the uterus. The puppies may also get affected with roundworms, as they breastfeed. Ingestion of affected animal or soil contaminated with roundworm eggs is also a cause of roundworm infestation. Once the dogs get infested, they will be overeating for sometime, but, after that, they will lose interest in food. Dogs with roundworms are often found to develop a pot-bellied appearance. Other roundworm symptoms in dogs include lethargy, loose and/or bloody stools, vomiting and breathing problems.
Hookworms: Hookworms are parasites with hook-like structures, which they use for getting attached to the intestinal walls (of the host), from where they suck blood. As in case of roundworms, hookworms too can be transferred to the fetus, while inside the uterus of the infected mother. They may also get transferred to the puppy, through breastfeeding. The larvae of hookworms are capable of penetrating the skin of the host and enter the latters body. Ingestion of larvae or eggs through contaminated water or soil is also a cause for hookworms in dogs. As these parasites suck blood, by attaching to the walls of the intestine, the dog will develop blood loss and resultant anemia, black tarry stools, vomiting, diarrhea and fatigue.
Whipworms: As far as whipworms are concerned, the most common method of transmission is ingestion of contaminated food, water and soil. These parasites too attach themselves to the intestinal wall of the host, sucking blood. In small numbers, they may not cause any symptoms, but, large infestation may cause inflammation of the intestinal surfaces. The affected animal may have mucus in the stools and may also develop anemia, weight loss, vomiting and diarrhea.
Tapeworms: They are parasites that require an intermediate host that acts as a vector. In case of tapeworms, such hosts include pigs, sheep and fish. Tapeworm infestation in dogs is mainly caused by ingestion of infected fleas that carry tapeworm eggs. Even ingestion of other animals that are infested with fleas or tapeworms may result in transmission of tapeworms. Symptoms of tapeworm infestation include restlessness and vomiting. The stools of the affected dog may contain white rice-like grains that are actually segments of tapeworms. Such segments may be found in and around the anal area of the animal. The animal may also start scooting, due to the irritation caused by these worms from other dogs.
Heartworms: In case of heartworms, mosquitoes are the vectors. These insects carry the larvae of heartworms and transmit the same to dogs, through bites. Once infested, heartworms multiply in numbers and settle down in the heart and the blood vessels. In large numbers, these worms can cause severe symptoms and may also result in death of the animal. The onset of the symptoms start with a cough. The animal will soon develop chest pain too. These parasites can also affect the vital organs of the dog, as they block the blood vessels.
Apart from the above mentioned methods of dog worms transmission, mutual grooming is also a cause for transmission of dog worms, especially, tapeworms. Even ingestion of the stools of other animals and ingestion of uncooked meat may result in dog worms. Prevention and treatment of dog worms is one of the vital aspect of dog care. So, regular monitoring is very much necessary to prevent and cure the condition. You may also consult the vet and opt for vaccinations in order to prevent dog worms. Timely diagnosis and treatment will prove useful in reducing the discomfort of the animal and also in preventing complications associated with dog worm infestation.

How to Get Rid of Bad Dog Breath
How to Get Rid of Bad Dog Breath
Has your dogs breath been smelling more than usual lately? Halitosis or bad breath is a common problem in dogs. There are many reasons that can lead to problem of bad breath in dogs. While sometimes the duration for which bad breath in dogs persists is a short one, a long duration suggests a deep rooted problem which needs proper attention and a trip to the veterinarian. In this article, we give the reasons for bad breath in dogs and tell you what you can do to get rid of stinky dog breath.
Reasons for Bad Dog Breath
Of the many health problems in dogs, bad breath is one of the most common issues. What causes bad breath in dogs? Given below are some of the main reasons for bad breath in dogs.
- One of the most common reasons is poor digestion. In case a dog is unable to digest his food properly, then it is possible that the dog may suffer from bad breath, especially dog breath that has the stench of fish.
- Build up of plaque and tartar may also cause bad breath or halitosis. Plaque is a result of deposition of bacteria, minerals, and decomposed food on the dogs teeth.
- Other periodontal diseases like tooth and gum problems could also lead to the problem of bad breath in dogs.
How to Get Rid of Your Dogs Bad Breath
Once you know what is the cause of breath problems, how do you get rid of bad breath problem that your dog is plagued by? There are couple of things that you can do at home that are a part of general dog care that will ensure that bad dog breath is not a common occurrence in your dog.
Proper Dental Care
One of the most important things is to ensure proper dental care for dogs. Brush your dogs teeth as often as possible. A general disregard for oral care may be responsible for the bad breath problem. Use a toothpaste specifically manufactured for dogs, and use a soft bristled toothbrush. If your dog does not allow you to use the toothbrush properly, use a finger brush. A finger brush can be easily worn over ones finger. Scrubbing your dogs teeth with baking soda may also help you reduce the plaque and bacteria problems. But use baking soda as one of the remedies for bad dog breath only if the toothpaste you are using is not making any difference to the oral hygiene problems. Giving your pet dog breath mints for dogs or using a dog breath spray for him may also do the trick where the problem at hand is concerned. Most of these products work in such a way that they do not allow a build up of tartar or plaque. These dog products are available in different flavors and you can opt to use for one that your dog seems to like.
Proper Diet
While there are home remedies for dog breath problems, it is always a good idea to check if you need to change the diet that you are giving your dog. It may be possible that the raw meat or canned food that you are feeding your dog is causing bad breath problems. Also keep an eye out to ensure that your dog is not eating out of the garbage can. Remember that the food you eat is not suitable for your dog. Ensure that you give him food that is healthy, digestible easily, and also nutritious. Change the brand of dog food that you feed your dog. That may be the cause of the bad breath problems. It may be a good idea to add fresh vegetables with a crunch to your pets diet especially vegetables like carrots, cucumbers, zucchinis etc. These vegetables help you get rid of the particles of food that often get stuck in their teeth.
If the problems of bad breath continue to persist after you have tried all the methods to get rid of bad dog breath, then it may be a good idea to take your dog to the vet for a thorough check up. Many health problems in dogs often first show up in the form of minor symptoms like bad breath. Proper dog care and regular visits to the vet are essential to ensure that your dog is healthy.

How to Get Your Dog to Sleep Through the Night
How to Get Your Dog to Sleep Through the Night
Ever watched the classic animation movie - Lady and the Tramp? Remember the scene where they get the puppy into their house for the first time and keep her designated in her room? And then the way she keeps barking the whole night through cause she doesnt want to sleep alone? And how she manages to make her way into their room in the end? Cuteness. But only when it comes to watching it in animation...Not when it comes to living it yourself. No way. Then it really does become a problem. Imagine a dog that does not sleep the night through and does not let you sleep either. Yet, even if it lets you sleep, the effects that sleep deprivation can have on your dog are varied and of serious proportions. In that direction, it is required that you know how to get your dog to sleep through the night..for his peace and yours. In this following article we will focus on the ways in which you can make this happen.
Getting Your Dog to Sleep all Night - Tips and Tricks
It might be utterly frustrating for you to see that your dog is spending one sleepless night after the other. Why and what is keeping it up, youll wonder. And the answer might be in something simple that you might not have ever thought of. Thats why you need to read the following section to know the steps that can be taken in this order.
1 - Check for Changes
You know how it is - you sleep like a log when you are in your own bed, with the same setting and following a particular schedule. Its much the same with dogs. They like things to be the way they are, in that similar order, night after night. The first thing you do then is to check if there has been any change in their order. It could be something as small as changing the position of their bedding, or shifting their sleeping crate. Think of any change that could have brought on the sleepless nights. Then work towards negating that and restoring the same order that they are used to.
2 - Prepare the Bed
When putting the dog to sleep, ensure that the sleeping area is extremely comfortable for him. Use a soft bedding that he finds comforting. In case it is a puppy, it has been seen that placing a small clock with a rhythmic beat under the blanket gets them to sleep faster. Make sure that you do not change anything of the place and/or bedding that they sleep in. This is likely to disturb their pattern and cause a problem as far as sleep is concerned.
3 - Its all in the Training
You might have planned for the dog to use a crate or sleep in a separate room, but he might have different plans. So then get yourselves on the same page. You need to train the dog to understand that. Train him on commands like sit and stay so that he knows when it is required to retire for the night. Also, do not ever let him get into the habit of barking and yelling and then you proceeding to open the door. Hell associate the two and then continue to do the same whenever he feels like it. That is why there has to be a proper schedule put into place. This includes, more than anything else, proper potty training. (Well get to this in more detail in the specified pointer that is to follow)
4 - Exercising is Important
Get your dog really tired before bedtime and it will sleep better. If the dog sleeps in the evening, then it is obvious that he will not be sleepy at night. So make sure that this does not happen. In addition to that, if there is plenty of exercise provided for, the dog will be really tired and go off into a deep, undisturbed sleep.
5 - Feeding Hours
It is essential that you plan your dogs feeding schedule well. When a dog has its food, it gets a sudden bout of energy which can lead to hyperactivity in dogs. And when there is energy coursing through his body, it will drive away the sleep. Make sure that you feed him at least 3 hours before his bedtime so that the food is well digested. Another reason for why you should not feed it right before bed is that he will want to do his business in odd hours of the night. This is one of the most important and usually ignored factor that you should look into.
6 - Potty Training a Dog
It is very, very important that you potty train your dog well. You also need to understand that a dog has to do its business many times a night, especially when it is a puppy. The way around this is to work out a schedule for yourself. Put the alarm for every 2-3 hours at night and take him out for his business. Do not wait for him to start yelping and barking for you to take him out. This will only help to strengthen the association of how when it barks, you come to the rescue. Which can be a problem for you later. Once the dog has become older, take it out some time before bed so that it is comfortable when it goes to sleep.
Now that you have some tips and tricks on getting your dog to sleep for the whole night, you will be better prepared to handle the situation at hand. Dont worry too much about it. Just be patient and encouraging and a sleepy dog will greet you all night through...Night after night.

Why do music shows hold so much power when they only get 1 viewer ratings
Why do music shows hold so much power when they only get 1 viewer ratings

Article: How do music shows remain so powerful even with only 1% in viewer ratings?
Source: Sports Donga via Nate
1. [+184, -10] To regular viewers, it may seem like groups just come on to perform a song or two before leaving but these groups actually have to be there by early morning and are on standby for an indefinite amount of time until recording and the live broadcast. Then they have to stay for the live just to clap for everyone before leaving. The reason viewer ratings for music shows are so low is because fans dont have to actually watch the show to watch everyone else to wait for their favorite artist to perform. They can just grab a video link after the performance.
2. [+159, -4] The shows may not matter to popular groups but rookies or artists from weaker agencies need to pander to these music shows and its so sad... They also wait up to 12 hours for these shows just to perform for a couple of minutes on stage. Even then, viewers dont watch anymore because its easier to watch performance clips on Naver than sit through it all. Furthermore, music shows are heavily based around idols so people who arent interested in that wont watch.
3. [+106, -3] I just find the entire concept of music shows so boring. Having two MCs try so hard to be funny when theyre not at all is annoying.
4. [+17, -0] There used to be ballads and hip hop and dance performances back then... but now its just the same old sounding songs... ??
5. [+15, -3] SBS Inkigayo is the strangest for me.. theyre so biased towards YG, why is that???
6. [+12, -0] Music shows have always been known for abusing their power. Dont want to go on IAC? Well you can kiss your Music Core slot goodbye. If you piss any of them off, you can forget about being able to perform a full three minutes of your song ??? I also cant stand the camera work on these shows. Back and forth, swirling around ?? Zooming in on the wrong member when someone else is singing... what is the point of having 3-4 rehearsals when they just broadcast the first one anyway... ???
7. [+8, -0] These shows obviously put agencies at a loss but they have no option but to put them on them. I didnt know before but I found out that agencies have to pay for the stage setups too which is why theres such a stark difference in stage quality between all of the groups. That expense on top of all the hair, make up, different outfits for every stage, the fact that outfits are also produced by their own stylists or are luxury brands, on top of the 10+ hours of waiting so you cant schedule other activities...
8. [+8, -2] SM is even more powerful ? Their rookies get the best recording schedule and get to perform two songs while other nugu agencies have to be there by early morning just to wait around to perform a 2 minute song.
9. [+8, -0] No one watches unless youre a fan. Poor sound quality and the camera work is head ache inducing.
10. [+8, -0] Video uploading has become so much faster that you can pick and choose who you want to watch right after the show so theres no reason to watch the live broadcast

Dont Get Caught Without Your Sunscreen
Doctor says many people dont know how much to use

By Robert Preidt
Sunday, June 12, 2016SUNDAY, June 12, 2016 (HealthDay News) -- Heading out on a warm, sunny day? Dont forget to cover up -- with sunscreen.
"Everybody -- and I mean everybody -- needs sunscreen," Dr. Jennifer Caudle a family medicine physician at Rowan University School of Osteopathic Medicine in Stanford, N.J., said in a university news release.
"Your ethnicity doesnt matter; how easily you tan doesnt matter. If you go out in the sun without sunscreen you are putting yourself at risk for melanoma, one of the deadliest forms of cancer," she warned.
Sunscreens carry sun protection factor (SPF) numbers, which indicate how much protection they offer against the suns ultraviolet radiation. The higher the SPF, the greater the protection.
A broad spectrum sunscreen protects against both ultraviolet A and ultraviolet B, both of which can damage the skin and contribute to skin cancer, Caudle explained.
"And a lot of factors can affect sunscreen performance. Swimming and sweating can wash off some sunscreens and most people dont use enough or reapply sunscreen often enough," she said.
Many people dont know how much sunscreen to apply.
"On average, about one ounce. That may not seem like a lot, but its enough to cover your palm or fill a shot glass," Caudle said.
She added that you should reapply sunscreen at least every two hours for maximum protection even if the sunscreen label advises differently.
