Boston Cataract Surgeon & Specialist Francis D’Ambrosio, Jr., M.D. shared information about patient expectations of cataract surgery recovery. “At D’Ambrosio Eye Care and The Cataract & Laser Center of Gardner we routinely perform cataract removal and lens implants using a very tiny incision, with no needles and usually only eye drops for anesthesia,” explained Dr. D’Ambrosio. “For the great majority of our patients the postoperative course is very comfortable and unremarkable. Typically, most patients dont require pain medications and at most have a slight foreign body feeling from the incision which usually clears in a few hours. The vision the next day can range from being excellent or at times a bit cloudy, depending on the amount of residual swelling in the cornea. Patients with mild cataracts generally have very little swelling. Very dense cataracts require more energy to remove, and this can produce more swelling in the cornea. This swelling usually clears quickly and most patients see quite well in a few days,” Dr. D’Ambrosio further explained. “Depending on the type of lens implant you have had there may be a little adaptation. For example, if you have had a near vision presbyopia correcting multifocal lens implant there may be re-learning of how to see far and near. Most patients adapt pretty quickly.”

Boston Cataract Surgeon, Francis D’Ambrosio Jr., M.D. explained, “The cost of cataract surgeryand lens implants can get a little confusing. Almost all insurances and Medicare cover the bulk of the cost of cataract surgery and basic lens implants. HOWEVER, deductibles, co-pays and the extent to which certain services are or are not covered can affect the actual amount you might need to pay out of pocket.” 

Here is some helpful information that will help patients better understand cataract surgery and lens implant cost.

Preoperative Testing and Consultation


With the exception of co-pays and deductibles specific to your insurance plan the eye examination, consultation and measurements necessary for cataract surgery and basic Intraocular Lens (IOL) implantation are covered services. If there are specialized tests that might be required such as more precise types of testing for multifocal lens implants or toric lens implants, you may need to pay for these tests out of pocket as your insurance and Medicare may only cover the basic testing. If this is the case, these extra tests may range in cost up to a few or several hundred dollars depending on the scope of the necessary testing.



Surgery Center Facility Fee


The surgery center facility fee is generally covered by insurances and Medicare although there may be a small co-pay or deductible with certain plans. This fee will cover the use of the facility including the operating room, the nursing staff, the instrumentation and equipment and all related operating room supplies. Depending on the types of anesthesia used it is possible that there will be an additional fee-mostly covered by your insurance or Medicare-for the administration of anesthesia and the staff.



Cataract Surgeon’s Fee
The cataract surgeon’s fee is generally covered by insurances and Medicare although there may be a small co-pay or deductible with certain plans.



Lens Implant Fee


The cost of a basic Monofocal lens implant lens implant is generally covered by insurances and Medicare within the facility fee. However, should you and your cataract surgeon elect to use an astigmatism correcting toric lens implant or multifocal lens implant there will be an out of pocket fee as most insurances and Medicare do not cover these more advanced types of lens implants.

Massachusetts Cataract Surgeon Francis D’Ambrosio, Jr., M.D. explained that cataracts and age related macular degeneration (AMD) share at least one common risk factor-AGE. Cataract formation and macular degeneration both increase in frequency and incidence with age and thus it is pretty common to have one, the other or both as we head through our 50’s, 60’s and 70’s. The question that many patients get confronted with is whether to have cataract surgery if they have macular degeneration. And, if they do, will it make the AMD get worse and faster.


The general consensus among cataract surgeons is that with proper preoperative diagnostic testing, examination and consultation patients who have age related macular degeneration (ARMD) and who develop a cataract are able to have cataract surgery after being counseled on what to expect with regard to overall vision improvements and lifestyle benefits. While ARMD is not a reason to avoid cataract surgery, realistic expectations must be established by and between you and your cataract surgeon. Further, the most current and widely accepted studies on AMD progression after cataract surgery found that there was no difference in the frequency of the disease among patients who had cataract surgery as compared to those who did not.

Recovery After Cataract Surgery


Cataract surgery today is really very advanced and quite “patient friendly.” At D’Ambrosio Eye Care we perform our Cataract Surgery and Lens Implants at the Cataract and Laser center of Gardner. This eye surgery facility is very comfortable and efficient and your entire Cataract Surgery procedure should take only 2-3 hours from the time you check in until the time you check out.

Second, advanced cataract surgery is most often performed with local or topical anesthesia. Thus, there is really minimal if any “recovery” from the anesthesia used. You are usually feeling “back to normal” by the end of the day of your surgery or certainly by the next morning.

Third, with the advanced Cataract Surgery technique of “small incision phacoemulsification” there is only a need to create a tiny incision-often just a few millimeters-through which the Cataract is removed and the intraocular lens is implanted. Thus the wound healing is very quick.

Finally, depending on the type of lens implant that is used it is highly likely that your distance vision will be dramatically improved within 24 hours. In fact many patients actually are comfortable driving the next day after their cataract operation. If you have a multifocal or accommodating lens implant to correct both your distance and near vision, it might take a bit longer to appreciate the full clarity of the near vision correction. So, it is pretty likely that you will be back to work in a few days. Depending on your cataract surgeon’s instructions, you should be able to resume all of your normal activities with a couple of days.

Boston Cataract Surgeon Francis D’Ambrosio, Jr., M.D. explained why cataract surgery patients often experience Posterior Vitreous Detachment (PVD) after a cataract operation. “Posterior Vitreous Detachment (PVD) does indeed appear to be more common in individuals who have had cataract surgery. There may not be an obvious "cause and effect" relationship but there are a number of possible factors that contribute to this,” said Dr. D’Ambrosio, Jr. of D’Ambrosio Eye Care with offices in Athol, Lancaster, Acton & Gardner, Massachusetts. 
  • The vitreous changes that lead to vitreous liquefication and instability and ultimately PVD are age-related, as are cataracts and the need for cataract surgery. 
  • Removal of a cataract can allow mechanical space shifts in the vitreous gel and perhaps increase the odds of PVD. 
  • Biochemical changes actually occur following cataract surgery and these appear to increase the likelihood of PVD, particularly if the posterior lens capsule is opened. 
  • Unusual complications of cataract surgery can disrupt the integrity of the vitreous gel and increase the odds of PVD. 
“What we know clinically as a practical matter is that it is pretty typical for some 50% or more of patients undergoing cataract surgery will experience a Posterior Vitreous Detachment (PVD), with some patients noticing visual symptoms including a “cobweb” or some floaters,” explained Dr. D’Ambrosio, Jr. “If the visual symptoms become excessive or are accompanied by a curtain or veil in your vision, or perhaps distortion of your vision, we want to hear from you immediately.”

Boston Cataract Surgeon Francis D’Ambrosio, Jr. M.D. shared information and facts about cataract surgery and diabetes. “With the proper detailed preoperative evaluation, examination and surgical care, cataract surgery can be successfully performed for patients with diabetes,” stated Dr. D’Ambrosio, Jr. “This is fortunate as diabetes is a well known risk factor for developing cataracts. Diabetic patients are also at risk for developing diabetic retinopathy, a serious threat to vision and a leading cause of new blindness. In fact, it is not uncommon for diabetic patients to need cataract surgery often at an earlier age than others who are not diabetic due to their increased risk factors,” he further explained.

Even though diabetes is known to predispose patients to slower and more difficult wound healing, the type of small-incision cataract surgery we use on a routine basis poses no additional risk to diabetic patients than non-diabetic patients. Often, it is important that diabetics considering cataract surgery have a careful evaluation of the retina-especially the macula-using ocular coherence tomography (OCT) to identify the presence of diabetic macular edema (DME), a known finding in diabetic retinopathy that can cause less then optimal results after cataract surgery. In addition, in certain situations cataract surgeons may take an added precaution of placing an injection of VEGF inhibitor such as Avastin™ or Lucentis™ into the vitreous of the eye to prevent new blood vessel growth. Other than these considerations diabetic cataract surgery can offer patients good results. Early diagnosis and treatment is the key to preserving eye health and vision with diabetes. I could write so much but had to stop myself. I feel like a brand new person as Im able to see colors and read road signs etc. Watch out mall, here I come. The moment I walked into DAmbrosio eye center for my first eye examination, I knew I made the correct choice. Everyone in the office was not only professional but treated their clients with respect, warmth and friendliness. I felt the same way when I entered the Cataract & Laser Center. I felt welcomed and was treated with the utmost respect. I was greeted with a smiley, friendly receptionist that took some necessary information. She put me at ease immediately. 

I went in the next phase, received my pretty yellow robe along with eye drops and a blood pressure reading, with yet another friendly face, that prepared me for the next step. She explained everything to me and what to expect next. 

Once I was seated, in the most comfortable recliners I ever sat in, several more procedures took place by caring, compassionate nurses that explained every step as they went along. They also made sure I was comfortable and doing okay. I could also ask questions through any of these steps. The last step, surgery itself went smoothly and Dr. Robbins is my new angel. She did a wonderful job. I was comfortable with a nice warm blanket and time went by pretty fast. After surgery, I went back to the recliners and received my instructions and then on my way. I was offered something to drink, eat etc. 

Later that evening Dr. Robbins checked in to see if I was doing okay and to let me know not to hesitate to contact the center, in the event something didnt seem right to me at any moment. 

My experience was wonderful. I felt I was treated extremely well by people that cared and made sure this experience was the best it could be. 

When I first considered eye surgery, I could just barely see; mostly shadows as my eyes were getting progressively worse. I am so amazed that I can even read the smallest type on the eye charts. After my second eye surgery, I was able to see even better. I only need to wear readers for small print.

My husband, who came along with me through the entire experience, is totally pleased with the outcome of the surgery and the professional, courteous treatment he also received during this time.

Im totally pleased with the eye surgery, the special care I was given, along with the clean friendly environment. The total experience was excellent and I certainly would recommend anyone to go there.

Boston Cataract Surgeon and Corneal Specialist JoAnn C. Chang, M.D. commented on cataract surgery complication. “Although cataract surgery complications are really pretty rare everyone with a cataract who is thinking about having eye surgery always has a concern about what could go wrong,” commented Dr. Chang.  Certainly there are things that could go wrong. “Basically in the hands of an experienced cataract surgeon using modern cataract surgery techniques we do everything possible to reduce the risk of complications, said Boston Cataract Surgeon JoAnn C. Chang, M.D. at D’Ambrosio Eye Care. “When you consider the huge benefit that patients receive in not only in terms of vision but also lifestyle, comfort, convenience and safety the risk of complications is really quite low”, said Dr. Chang. For routine cataract surgery in healthy patients the complication rate is well under 5%. This might increase somewhat if you have diabetic eye disease, glaucoma or other types of health or eye problems.

So what kinds of things can go wrong? First there are a handful of things that can happen during or within the first 24-48 hours after a cataract operation. These include the very unusual things like bleeding inside the eye which is really rare, a little bleeding on the surface of the eye or an inner eye infection which is quite rare. Sometimes, if we need to anesthetize the eye using an injection you may have a little “black and blue” eye or an eyelid bruise. This usually clears up pretty quickly. Sometimes the “capsule” around the crystalline lens that is removed as part of the operation might be broken during the surgery and we might need to do a “vitrectomy” as part of the surgery to get a good result. This too is pretty unusual. More common in very nearsighted patients, some patients who have cataract surgery can have a retinal detachment. Occasionally in patients who have had “no stitch” cataract surgery the actual incision may leak. If this happens it is sometimes necessary for us to put a stitch in place to tightly close the incision. Then there are a few types of complications that can occur over a longer period of time. Very rarely the intraocular lens implant can move off center or the center of the retina can swell. By far the most common complication is a clouding of the lens capsule that has been left in place to support the lens implant. Fortunately, this complication is pretty easily managed with a quick, painless procedure called a YAG Laser Capsulotomy that restores the vision almost immediately after the procedure.

Although it might sound like there are lots of possible complications of cataract surgery, rest assured they are quite unusual. This most common one of capsule opacity is easily treated.  If you need cataract surgery it is important to have a little knowledge so you appreciate the degree of skill and experience required of your cataract surgeon to help you get the best result.

I recently had cataract surgery on both of my eyes. I could not be happier with the procedure.  Immediately after each eye was done I feel like my vision went from very poor to as if I never had vision problems to begin with.  As of now, I only need reading glasses, where before I had this amazing cataract surgery I had to wear glasses all the time -- day and night. Massachusetts Cataract Surgeon Francis D’Ambrosio, Jr., M.D. commented on correcting astigmatism after cataract surgery with toric lens implants. “Most cataract patients know from their eye exams or eyeglass prescriptions that they have some amount of astigmatism. However, sometimes they don’t realize that it is important for their cataract surgeon to correct that astigmatism in order to get the best visual results after cataract surgery,” explained Dr. D’Ambrosio of D’Ambrosio Eye Care with offices in Athol, Lancaster, Acton & Gardner.

Astigmatism is a common refractive eye condition that that is most often due to an uneven curvature of the cornea. If the cornea were perfectly shaped, it would have a spherical round even shape, like a basketball. In eyes that have astigmatism, the cornea is typically curved more like an oblong, football-like shape, similar to the shape of the back of a teaspoon. “Depending on the degree and orientation of the unequal curvatures, astigmatism can cause mild or even severe blurry vision,” explained Dr. D’Ambrosio.

Patients with cataracts and who are in need of cataract surgery have three choices when considering how to correct their astigmatism.

First, patients can simply choose to be dependent on wearing eyeglasses after cataract surgery in order to correct astigmatism.

Second, if appropriate, some patients may opt to have an additional surgical procedure called Limbal Relaxing Incisions (LRI) or third, patients can elect have an astigmatism correcting toric lens implant in which the astigmatism correction is incorporated into the lens prescription just as in eyeglasses. “Toric lens implants are placed in the eye in exactly the same manner as ordinary lens implants at the time of cataract surgery and correct the astigmatism as part of the cataract surgery operation,” further explained Dr. D’Ambrosio.

Boston eye doctor Francis D’Ambrosio Jr., M.D. commented on interesting research regarding cataracts and ovarian cancer risk. “As an ophthalmologist, any association between eye diseases, problems or conditions and other health problems is of great interest to me,” stated Dr. D’Ambrosio Jr. “There may be a relationship whereby women who develop cataracts are less prone to developing ovarian cancer, according to researchers."

It is widely recognized that the major health benefit of exposure to Ultraviolet (UV) Radiation from the sun is the production of Vitamin D, which is helpful in protection against several human cancers, including ovarian carcinoma. On the other hand, Ultraviolet (UV) Radiation from the sun is a recognized risk factor for cataract development. Researchers reporting in the November 2011 publication Cancer Epidemiology, Biomarkers & Prevention studied 709 women with primary invasive ovarian carcinoma and 1101 controls to examine the association of ovarian carcinoma risk with a history of cataract.

Some interesting findings were that older age, a history of Type 2 Diabetes and skin cancer were significant risk factors for developing a cataract. Further, a history of cataract was reported by 14% of cases and 17% of controls and was significantly associated with a reduced ovarian carcinoma risk. These findings add indirect evidence to the hypothesis that lifetime Vitamin D exposure may be inversely associated with risk of ovarian carcinoma. Additional studies are needed to further investigate the potential behavioral and biological factors that might influence association of cataract with ovarian cancer.

Monovision Cataract Surgery is a technique that is used to help patients increase their range of clear vision after a cataract operation. The technique of monovision has been used for many years by contact lens wearers as well as patients with presbyopia having laser eye surgery for the Laser Vision Correction of nearsightedness, farsightedness and astigmatism. 

The monovision technique is simply adjusting the optical correction in each eye so that the dominant eye is corrected for clear distance vision and the non-dominant eye is corrected for improving near or intermediate range vision. For patients who have been wearing monovision contact lenses successfully, or who have had monovision LASIK and are pleased with the vision, taking the step into monovision cataract surgery is a natural progression. For others, they must consider the possible benefits as compared to those provided by near vision presbyopia correcting multifocal lens implants as some 30% of monovision patients fail to adapt adequately to the “blended” vision. 

Researchers reporting in the May 2011 Journal of Refractive Surgery studied the optimal monovision prescription that should be used in order to get the highest levels of patient acceptance and overall visual acuity and depth perception. They concluded that the prescription range of 1.00 D-2.00 D of monovision correction was the maximum range and that the optimal prescription for monovision cataract surgery was 1.5 D. While this is useful information, patients should understand that a 1.5 D near correction may very well be insufficient for them to perform their daily tasks comfortably without needing readers or bifocals.

Boston Cataract Surgeon Francis D’Ambrosio, Jr., M.D., shared thoughts about waiting too long to have cataract surgery and lens implants. “The decision of when to have a cataract removed and replaced with lens implants is really a very personal decision,” said Dr. D’Ambrosio, Jr. “Some patients are simply not comfortable with the slightest degree of blurry vision or glare sensitivity, whereas others surprise me with how tolerant they are. So, I generally tell patients that their cataracts should be removed when they feel that they have visual discomfort or inconvenience-or any safety issues with navigating each day’s activities with confidence,” Dr. D’Ambrosio, Jr. of D’Ambrosio Eye Care with offices in Athol, Lancaster, Acton & Gardner, Massachusetts explained.

What patients should know though is that waiting too long for cataract surgery can result is some unintended consequences and possible risks. “For example, as a cataract matures, it does become denser and somewhat more difficult to remove. This requires more energy and may lead to prolonged inflammation or even intraoperative complications,” stated Dr. D’Ambrosio, Jr. “For certain we don’t want the cataract to become so dense, swollen and cloudy that it causes a type of glaucoma or impair our view of the retina,” Dr. D’Ambrosio, Jr. stressed. “In these instances your cataract surgeon is likely to recommend proceeding with cataract removal even though you may not think you are quite ready.”

Boston Cataract Surgeon Francis D’Ambrosio, Jr., M.D. reviewed guidelines for resuming exercise after cataract surgery and lens implants. “The cataract surgery and lens implant operation allows for a much quicker practical and visual recovery,” stated Dr. D’Ambrosio, Jr., of D’Ambrosio Eye Care with offices in Athol, Lancaster, Acton & Gardner, Massachusetts. “This is important to so many patients-especially as they want to be able to resume their daily activities and lifestyle quickly. One area we always discuss with patients is how long they need to wait or restrict their exercise routine,” Dr. Dr. D’Ambrosio, Jr. further explained. “Certainly exercise guidelines may vary slightly depending on your cataract surgeon and on the specific details of your cataract surgery, but in general within 24 to 48 hours you should be able to do light walking. Avoid any heavy lifting or bending from the waist to get objects from the floor until you get approval to do so from your cataract surgeon. Swimming and water activities should be avoided for at least the first week after surgery because getting water in your eyes could increase the risk for infection. However, most patients are completely back to their exercise routines within one month after surgery.”
